Take a Motorcycle training or Refresher course, use this form along with the required attachments and receive a Patch, Pin and Gift Certificate!

This should be carried by all Chapter Road Captains and be made available at any Chapter Sponsored Events. Completion of this form is required by all adult non-chapter members to participate in a HOG® sanctioned event or ride.
